How to Register with the Surgery

You can register with the surgery in one of two ways:

1. Visit the Surgery
   • You can visit the surgery in person and ask for a paper registration form. Fill it out and hand it back to a member of staff at reception.
2. Register Online
   • You can register online by visiting https://gp-registration.nhs.uk/P81169.
   • After registering online, you will also need to complete the New Patient Agreement Form.

Please note: We only accept registrations from patients within our catchment area. You can check if you live within our area by using the map above.
