We are not able to provide the following services
- Completing ANY forms for sporting activity (skydiving, bungee, scuba diving, etc.)

- Private blood tests
- Housing letters
- Supporting letters for benefits (Universal Credit etc.)
- Lasting Power of Attorney/COP3 – solicitors or social worker can complete and patients themselves can make, register or end a lasting power of attorney – https://www.gov.uk/power-of-attorney/make-lasting-power
- Confirmation letters of address/residence or ID (e.g. for banks etc.)
- Letters stating anything that is not supported by the patients’ medical record
